New Patient Forms
New patients forms for functional health consults can be downloaded in PDF form below and filled out before coming into the initial visit.
Patients coming in for treatment for musculoskeletal complaints can fill out intake paperwork prior to the initial visit.
Payment
Our fees are set up to provide the care our patients need at prices they are able to afford. As a result of our fee structure, most of our patients pay at the time of the visit using cash, check, or credit card. We accept Visa and Master Card.
We are out of network with all insurance companies, but we are happy to file your claim for you. Any out-of-network benefits will be reimbursed directly to you.
We do see Medicare patients, but we are not a participating Medicare provider. This means that Medicare patients pay at the time of the visit, and any reimbursement is delivered directly to you.
Pricing
Chiropractic Appointments
Initial Exam Fee — $110 (Includes initial exam and adjustment)
Adjustment — $60 (with payment at time of service)
Acupuncture — $35
Chiropractic Adjustment with Acupuncture — $80
Functional Health Appointment
Appointment Price — $150 for ~1 hour long appointment
Labs and nutrition are priced separately, but these are determined on an ‘as needed’ basis and will be discussed with patients prior to recommendation.
